The website is intended for informational and educational purposes. It is not intended to provide individualized medical advice, diagnosis, treatment, or emergency assistance.
2. No Medical Advice
Information presented on this website is general in nature and should not be interpreted as medical advice for any particular individual or condition.
Website content does not replace:
An examination by a qualified healthcare professional
A personalized diagnosis
An individualized treatment plan
Informed consent
Medical testing
Advice from a physician who is familiar with your medical history
Do not begin, stop, delay, or modify any medication, supplement, treatment, diet, or healthcare plan based solely on information found on this website.
Always consult an appropriately qualified healthcare professional regarding questions about your health, symptoms, diagnosis, medications, or treatment options.
3. No Physician-Patient Relationship
Accessing this website, reading its content, submitting a form, sending an email, calling the Practice, or requesting a consultation does not create a physician-patient relationship.
A physician-patient relationship is established only after the Practice has:
Accepted you as a patient;
Completed an appropriate consultation or medical evaluation; and
Obtained any required registration documents, consent forms, and agreements.
The Practice is not obligated to accept any person as a patient merely because that person submitted a consultation request or contacted the office.
Until a physician-patient relationship has been formally established, you should continue to seek care from your current healthcare professionals.
4. Medical Emergencies
This website and its forms are not monitored as emergency communication systems.
Do not use the website, email, text messaging, consultation forms, or general contact forms to request emergency assistance or time-sensitive medical care.
Call 911 or go to the nearest emergency room when you believe you are experiencing a medical emergency.
Potential emergencies may include, but are not limited to:
Chest pain
Difficulty breathing
Severe bleeding
Loss of consciousness
Signs of a stroke
Severe allergic reactions
Suicidal thoughts
Sudden or rapidly worsening symptoms
Health Choices Now is not responsible for harm caused by using the website or electronic communications in place of emergency medical care.
5. Consultation Requests
The website may allow you to request a consultation or additional information about a treatment or service.
Submitting a consultation request:
Does not guarantee an appointment
Does not guarantee acceptance as a patient
Does not guarantee that a treatment is appropriate for you
Does not establish a physician-patient relationship
Does not constitute medical advice
Does not guarantee any particular health outcome
The Practice may contact you to obtain additional information, discuss scheduling, explain the consultation process, or determine whether the Practice may be able to address your needs.
The Practice reserves the right to decline or discontinue a consultation request when appropriate, subject to applicable law and professional obligations.
6. Information You Submit
You agree that information submitted through the website will be accurate, current, and complete to the best of your knowledge.
You should not submit false information, impersonate another person, or submit information about another individual without appropriate authority or permission.
Do not submit highly sensitive information through a general website form unless specifically instructed by the Practice. This may include:
Social Security numbers
Financial-account information
Government-issued identification numbers
Complete medical records
Detailed diagnostic reports
Highly sensitive photographs
Information requiring immediate medical attention
When detailed medical information is needed, the Practice may direct you to a more appropriate communication or records-submission method.
7. Email, Telephone, and Text Communications
When you provide your email address or telephone number, you authorize the Practice to contact you regarding your inquiry, consultation request, appointment, requested information, or services.
Electronic communications may include:
Email
Telephone calls
Voicemail
Text messages
Appointment reminders
Follow-up communications
Educational information you requested
Ordinary email and text messaging may not be completely secure. Do not use ordinary electronic communications for emergencies or to transmit highly sensitive information unless instructed by the Practice.
When promotional or newsletter communications are offered, you may unsubscribe using the link provided in the communication or by contacting the Practice.
Consent to receive promotional text messages is not a condition of receiving medical care. Message and data rates may apply. You may opt out of nonessential text communications by replying STOP when that option is provided.
8. Self-Pay Services and Fees
Health Choices Now operates as a self-pay medical practice and does not bill health plans.
Patients are responsible for paying the fees associated with consultations, treatments, products, laboratory services, medical supplies, and other services they authorize or receive.
Information displayed on the website may not include every fee or expense associated with a treatment program. Treatment costs may vary depending on factors such as:
The patient’s individual needs
The recommended treatment plan
The number or duration of visits
Laboratory testing
Medications or supplements
Medical supplies
Services provided by outside professionals or facilities
Any website pricing is subject to change unless it is included in a separate written agreement accepted by the Practice.
Before receiving care, patients may be required to review and accept separate financial policies, payment terms, cancellation policies, refund policies, and treatment agreements. Those separate documents will control when they conflict with general information presented on the website.
9. Treatment Information
Descriptions of treatments and services are provided for general educational purposes.
A treatment described on the website may not be:
Appropriate for every patient
Available to every person
Appropriate for every medical condition
Recommended without additional testing or evaluation
Available in every jurisdiction
Covered by the same regulatory status for every proposed use
The benefits, risks, alternatives, expected costs, and regulatory considerations associated with a treatment should be discussed directly with an appropriately qualified healthcare professional.
Website content does not constitute informed consent for a medical procedure or treatment. Separate informed-consent documents and discussions may be required before treatment.
10. No Guarantees of Results
Medical outcomes vary from person to person. Health Choices Now does not promise or guarantee that any treatment, service, medication, supplement, product, lifestyle change, or healthcare program will produce a particular result.
Factors affecting outcomes may include:
A patient’s diagnosis
Age and general health
Medical history
Severity and duration of the condition
Lifestyle
Treatment participation
Individual biological response
Other medications or treatments
Factors outside the Practice’s control
Statements describing possible benefits or previous results should not be interpreted as promises, warranties, or guarantees.
11. Testimonials and Patient Experiences
The website may include testimonials, patient stories, reviews, interviews, or descriptions of previous experiences.
These materials reflect individual experiences and may describe favorable or best-case outcomes. They do not represent, warrant, or guarantee that another patient will experience the same or similar results.
Testimonials should not be used as the sole basis for making a healthcare decision.
12. Educational and Professional Opinions
The website may contain medical commentary, professional opinions, clinical observations, summaries of published materials, or discussions of emerging and alternative approaches.
Medical knowledge and professional standards may change over time. Website materials may not reflect the most recent research, regulatory developments, or clinical guidance.
Nothing on the website should be interpreted as criticism of or a directive to disregard the advice of another qualified healthcare professional.
You are encouraged to discuss information obtained from the website with your existing physician or another appropriately qualified healthcare professional.
13. Accuracy and Updates
Health Choices Now makes reasonable efforts to provide useful information but does not warrant that website content will always be:
